Those swords might not be in the game for the players, but they are on the server files, so yeah they can appear.

So ignore whats inside that window of refine, there could be anything. The error itself is due to corrupted packet that has the header of a refine. It appears to be random and yes it appears on every server as it is a client flaw since the game was released. Thou is useful to know if this happens more frequently now than in the past.
